General Statement of Duties: The Family Medicine Physician at WSMC is responsible for providing direct outpatient medical services to WSMC patients.  S/he is responsible for rendering services that reflect current standards of medical care appropriate to the local medical community.  S/he is responsible for coordination of care as appropriate, and for documentation as appropriate for continuity of care and risk management.  S/he serves as a member of the health care team whose goals are described in the WSMC mission statement, and assumes responsibility as assigned for quality assurance/quality improvement efforts.

General Statement of Duties: The Licensed Vocational Nurse (LVN) at WSMC is a licensed staff member who performs those duties as outlined here. Under medical provider or RN direction, and operating within the scope of practice of an LVN, provides nursing care for clinic patients following established standards and practices. The LVN directs patient flow, sets a patient-friendly environment in the health center and provides direct patient care, assisting the medical provider by preparing the patient for the visit, taking appropriate vital signs, collecting requested specimens and preparing the patient for medical procedures. The LVN at the direction of the medical provider, also performs CLIA waived laboratory testing, EKG’s, draws blood, and gives immunizations and medications. They direct the patients flow by setting up medication refill requests, sorting and prepping paperwork and collecting and recording information for the provider.

 

MEDICAL ASSISTANT

General Statement of Duties: The Licensed Clinical Social Worker is responsible for providing brief therapy, solution focused, behavioral health interventions for medical clinical patients. Serves clients who frequently use emergency departments for non-urgent care, providing them with a healthcare home and the supportive services needed to establish treatment and prevention in the community. This includes conducting assessments and providing therapy to adults with any combination of mental health issues, substance-use disorders and medical complications. The Social Worker will develop behavioral health treatment plans integrated with the patient’s medical treatment plan and will provide consultation to physicians regarding mental health issues.
