General Statement of Duties:
The Family Medicine Physician at WSMC is responsible for providing direct outpatient medical services to WSMC patients.  S/he is responsible for rendering services that reflect current standards of medical care appropriate to the local medical community.  S/he is responsible for coordination of care as appropriate, and for documentation as appropriate for continuity of care and risk management.  S/he serves as a member of the health care team whose goals are described in the WSMC mission statement, and assumes responsibility as assigned for quality assurance/quality improvement efforts.
 
Education and/or Experience:
1. Knowledge: Licensed Physician (with hospital privileges in good standing preferred).
2. Education: A doctor of medicine or a doctor of osteopathy degree from an approved school of medicine or osteopathy, acceptable to the Surgeon General, HQ USAF, is mandatory.
3. Experience: Minimum of 2 years of experience in a hospital, medical group, or similar medical facility (preferred)
4. License: Current California Physician License, current DEA.
Additional Preferred Experience:
a. Familiarity with an electronic practice management system.
b. Computer literate (i.e. logging on to computer, usage of keyboard and mouse, familiarity with Windows, Microsoft Word and Microsoft Excel)

General Statement of Duties:
A Nurse Practitioner/Physician Assistant provides primary medical care for Western Sierra Medical Clinic, Inc. patients.
 
Education and/or Experience:
1. Valid California license to practice as a Nurse Practitioner/Physician Assistant.
2. Ability to do minor laboratory procedures including urinalysis, wet mounts, draw blood and do cultures.
3. X-ray technology license in California is desirable.
4. Minor surgery skills.
5. BLS Certified
Additional Preferred Experience:
a. Familiarity with an electronic practice management system.
b. Computer literate (i.e. logging on to computer, usage of keyboard and mouse, familiarity with Windows, Microsoft Word and Microsoft Excel).

General Statement of Duties:
The LVN Case Manager holds a key role in the support of primary care. This role has both a clinical (e.g., management of provider desktops, wound care, administering immunizations, etc.) and patient education component (e.g., proper use of insulin, healthy diet, how best to take and track medication use, etc.). The LVN Case Manager supports and coordinates individuals’ care through a variety of means including: providing information and assistance connecting to community, state or outside resources; educating patients related to their specific health state or preventative care needs; offering ongoing support and education to patients through scheduled calls or meetings; offering nursing and triage advice as appropriate within their scope of practice; keeping providers updated on the status of their patients and the resources and interventions offered; and monitoring recall systems as assigned to support patients in health monitoring.    
 
Education and/or Experience: 
1. Licensed Vocational Nurse in the State of California  
2. BLS for the Health Care Provider or equivalent CPR class is required.
3. Experience in a supervisory or quality assurance position preferred.
4. Basic knowledge of governmental (Medicare, Medi-Cal etc.) healthcare reimbursement systems is preferred.
5. Basic knowledge of post-acute healthcare resources such as: SNF, Home Health,
6. Hospice and their admission criteria is preferred.
