Chronic Disease Self Management - New Series starting December 1!

Free weekly workshops created to help people living with a variety of chronic health problems, including diabetes, cardiac conditions and chronic pain.  The Self-Management Program enhances regular treatment and helps participants discover skills to manage their health and remain as active and healthy as possible.

Learn how to:

  • deal with feelings of frustration, fatigue, pain and isolation
  • exercise to maintain and improve strength, flexibility, and endurance
  • use medications properly
  • communicate effectively with family, friends, and health professionals
  • obtain proper nutrition
  • evaluate new treatments

Each participant will recieve the companion book, Living a Healthy Life With Chronic Conditions, 3rd Edition, and an audio relaxation tape, Time for Healing.

Classes are 2 1/2 hours and held once a week for six weeks.  Classes are highly participative and provide mutual support for creating more active and fulfilling lives.

Daytime and evening courses are offered every two months.  Advanced registration is required to attend.

In addition to offering this workshop, WSMC has also offered a summit to "train the trainer" so more people in our community are qualified to offer a course in chronic disease self management. 

Weight Loss Support Group
Miners Family Health Center, Grass Valley:
This support group meets weekly and is led by a variety of group facilitators which provide a holistic view on weight loss through behavior modification, medical and health benefits of weight loss and nutrition.  For more information call Joanna Snider-Lipa at (530) 273-9339 x226.

Western Sierra Medical Clinic, Downieville:This support group meets the 1st Monday of each month from 12:30pm to 1:30pm.  The group facilitator is Danielle Yantis, Registered Dietician, and topics such as behavior modification, medical and health benefits of weight loss and nutrition.

Get-Fit Program for Corporations or Schools  
A weekly program that challenges participants to get-fit through a variety of educational and motivational resources provided by Registered Dietician, Danielle Yantis. Start a get-fit challenge at your school or business.
